Facility Evaluation Report

On 02/13/26 at 5PM, Licensing Program Analyst (LPA) D Panlilio conducted an unannounced Case Management visit to discuss the self-reported incident report dated 02/09/26. LPA met with administrator (ADM) and explained the purpose of the visit. LPA observed only one client residing at the facility.

ADM stated that on 02/07/26, one staff member was scheduled to work from 12AM to 8AM. However, staff were not present at the facility when C1 returned to the facility around 2:30AM after an outing. C1 stated she attempted to gain entry by pressing the doorbell multiple times, but no one responded. C1 also contacted the staff on duty by phone with not response. C1 called her boyfriend who called local police to assist C1 in entering the facility. Local police obtained facility door PIN from staff by phone who were scheduled to be on duty but were unable to return to the facility at that time. C1 was able to enter the facility at approximately 3:30AM. ADM conducted an internal investigation and terminated two staff involved in the incident.

Immediate civil penalty of $500 assessed during visit due to absence of supervision.

Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of correction (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
