Facility Evaluation Report
On 12/22/25 at 2:15PM, Licensing Program Analyst (LPA) D Panlilio conducted a case management visit and met with administrator (ADM). LPA explained the purpose of the visit with ADM.
At 02:20PM, LPA toured the facility including but not limited to four (4) bedrooms, office, 3 full bathrooms, dining room, living rooms, laundry room and kitchen. LPAs observed the facility is clean, odor free and in good repair. LPA also observed 4 clients live at the home.
ADM stated that effective 12/01/25, Regional Center of the East Bay (RCEB) has approved the facility to be operating under the Supported Living Services Program (SLS) for three (3) clients. One client will move to the other Connected Living facility # 079200968 located at 2350 1/2 Galloway Court Antioch CA 94509 under the same SLS program approved by RCEB.
At 2:42PM, ADM surrendered the facility's State License to LPA for facility closure.
No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
