Facility Evaluation Report

On 4/24/2025, at 1:40pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 4/10/2025. LPA met with Albert Vega, Program Director and explained the purpose of the visit.

The incident occurred on 4/08/2025, regarding C1 AWOL'g from program outing. S1 stated C1 there were ten (10) clients on the outing at the park with four (4) staff. Two of the clients requires a 1:1 staff during community outings. Interviews with S2, S3, S4, and S5 all stated each thought the other had C1. S1 stated program has new protocol when clients attend outings. Program let's one client out at a time, has a list in van for clients, and when clients returns from outings they are accounted for again. S1 also stated meeting was held on 4/08/2025, with all staff regarding incident.

LPA obtained outing sheet, C2's Individual Program Plan quarterly dated 10/8/2024. S1 stated C3 was approved for 1:1 but haven't received the document.
