Facility Evaluation Report

On 5/9/2025 at around 9:15AM, Licensing Program Analysts (LPAs) A Gomez and Y Brown arrived to conduct a Case Management Closure visit and met with Licensee Bernadette Dugan and explained the purpose of the visit.

During the visit, LPAs inspected the facility inside and out including but not limited to bedrooms, bathrooms, kitchen, dining area, living area and storage areas. LPAs observed that there were no clothes, shoes, or personal belongings in any bedrooms. There were no clients observed at the facility. Licensee states that all two (2) clients have been relocated to new placement.

Licensee sent notification to Community Care Licensing Division (CCLD) regarding closing facility and ceasing operation. Licensee refused to surrender their copy of the License to LPAs during the visit. Licensee states that they sent a copy of their to CCLD.

No Deficiencies cited during today's visit. Exit interview conducted and a copy of this report provided to Licensee.
